To date hypertrophic cardiomyopathy presenting in infancy has been associated with a poor prognosis, often progressing to severe congestive heart failure and death. Patients with findings indicative of a secondary form of hypertrophic cardiomyopathy were excluded from further analysis once this diagnosis was made. This case is illustrative, as previous reports have described steroid-induced hypertrophic obstructive cardiomyopathy as being transient and benign. It is also important to note that ventricular arrhythmias in infants and in children with hypertrophic cardiomyopathy are seen very infrequently. We currently recommend abstention from competitive or strenuous sports for the majority of patients with hypertrophic cardiomyopathy. Older age at diagnosis and the presence of arrhythmia were the only predictors of a poor outcome in pediatric patients with idiopathic dilated cardiomyopathy. It may also suggest a myocardial abnormality, such as dilated cardiomyopathy or myocardial non-compaction. Cardiac diagnoses included normal heart (n=16), status after correction of congenital heart disease (n= 13), and cardiomyopathy (n=3). Catheterization at 5.5 months of age revealed a non-obstructive form of hypertrophic cardiomyopathy with elevated left ventricular end diastolic pressures that regressed with verapamil. Initial diagnoses of the patients were congenital heart disease (n=l3), cardiomyopathy (n=9), cystic fibrosis (n=7) and primary pulmonary hypertension (n=l). In the subsequent 18 years of follow-up, evidence of biventricular cardiomyopathy became manifest. Right ventricular diastolic filling was also impaired in children with dilated cardiomyopathy. There was no evidence of old or recent myocarditis, nor hypertrophic cardiomyopathy. The infant reported here had a severe episode of hypoperfusion associated with the development of hypertrophic obstructive cardiomyopathy. In others, exercise capacity is limited and the risk is high (for example, severe hypertrophic obstructive cardiomyopathy). Of the many structural anomalies reported in the heart, hypertrophic cardiomyopathy has become recognized more frequently since the advent of echocardiography. The two patients with persistent cardiomyopathy on therapy died suddenly at 3.6 and 9.2 years respectively. Prolonged, high-dose dexamethasone treatment for bronchopulmonary dysplasia may be associated with the development of a transient form of hypertrophic cardiomyopathy. Would the same tendency be seen in ischaemic cardiomyopathy without myocardial infarction? The patient showed the unusual association of cardiomyopathy and neuroblastoma.
