The majority of operative procedures were arthroscopies (90.1% of all procedures), most of which were undertaken on a day-case basis. For most studies, however, the threshold was approximately 20% to 30% of averted arthroscopies. From a patient's perspective, diagnostic or therapeutic arthroscopies are virtually identical: both are typically day-case procedures involving similar recovery periods. Two patients had bilateral knee arthroscopies; the results reported here only refer to the worse knee. The remaining patients received conservative therapy and were not scheduled for arthroscopy within the period of observation. An additional four patients cancelled the arthroscopy and did not have the procedure performed by any of the physicians during the period of observation. For patients who had undergone an arthroscopy, additional data on preoperative diagnoses, procedures performed, and postoperative diagnoses were collected. Furthermore, we also assumed that the arthroscopic diagnosis had 100% accuracy, since arthroscopy has been considered as the gold standard. The arthroscopy report was not found for two patients, who were therefore excluded from the analysis. However, a substantial proportion of the therapeutic arthroscopies involved procedures of unclear efficacy, such as debridement or plica excision. Patients undergoing arthroscopy may have a more definite diagnosis of knee pathology, since their physicians have already decided to perform surgery on them. This finding may have had a particularly important impact on some of the key resource use parameters, in particular the use of arthroscopy.
