A chest x-ray showed a cardiothoracic ratio of 72% with clear lung fields. The electrocardiogram was normal and, on chest x-ray, the cardiothoracic ratio was unchanged. Postoperatively, the cardiothoracic ratio was significantly decreased in patients with (0.52 + 0.01) and in patients without (0.51 + 0.01) an additional source of pulmonary flow. A total of 38 (range 0-4) cardiothoracic operations were performed prior to transplantation, with a further three reoperations required for immediate postoperative problems. Recent changes in clinical practice have increasingly been directed by cost containment pressures from which cardiothoracic surgery is not immune (16).
