Caesarean section was associated with a decrease in the internal core of the urethral sphincter but not the rhabdosphincter. Urethral obstruction due to posterior urethral valves was the diagnosis in 14 of the 36 fetuses. These anatomical changes predispose elderly women towards hypermobility in the vesical and urethral base, as well as resulting in an intrinsic urethral weakness. Prior to the advent of prenatal diagnosis boys with urethral valves frequently presented with life-threatening sepsis in the first year of life and the condition carried an appreciable mortality rate. The urethral support mechanism is essential for continence and is dependent on the interaction of the bladder neck, pubo-urethral ligaments, urogenital diaphragm and the muscles of the pelvic diaphragm.
