All moodstabilizer drugs (including newer anticonvulsants) and all atypical antipsychotic drugs were available without formulary restriction. Despite the essentially uniform consensus supporting a "moodstabilizer first" approach to management of bipolar depression, not all psychiatrists follow this strategy. All were prescribed a typical (n=15) or an atypical neuroleptic (n= 32), 19 were prescribed an antidepressant, and 16 were prescribed a moodstabilizer. The sample had been on their present moodstabilizer for about 5 years, 85 % were taking lithium. Patients receiving a moodstabilizer plus adjunctive anti-depressant therapy had similar outcomes to patients receiving a moodstabilizer only. Four manic patients were in receipt of lithium carbonate, 11 were in receipt of another mood stabilizer, 18 were in receipt of a neuroleptic and one was prescribed an antidepressant. In the first year of the long-term follow-up phase 121/135 (90 %) of the subjects on whom the information was available received an antidepressant or moodstabilizer. Again, no advantage was observed for the antidepressant when compared with the moodstabilizer. In the case of the remitted patients, 11 were in receipt of lithium, one was receiving another mood stabilizer, five were prescribed a neuroleptic and six were taking antidepressants.
