In patients with severe ventricular systolic dysfunction, elevated serum noradrenaline is a sign of a bad prognosis independent of haemodynamic and ejectionfraction parameters. Thus, enddiastolic and end-systolic volumes are computed from their respective data sets and ejectionfraction is calculated from these values. Three-dimensional echocardiography also allows the calculation of ventricular volumes and ejection fraction without the need for geometric assumptions, since it reconstructs the entire ventricular cavity. Right ventricular ejectionfraction was calculated as stroke volume divided by end-diastolic volume. Estimates of right ventricular volume and ejectionfraction were compared to determinations made using magnetic resonance imaging. On first pass radionuclide ventriculography, mean right ventricular ejectionfraction was almost identical in the two groups. For calculation of ejectionfraction, the largest and smallest ventricular areas were taken as diastole and systole respectively. Normal right ventricular ejectionfraction at our institution (mean + 2 standard deviations) is 0.65 + 0.16 (range 0.49 to 0.81). The ejectionfraction is usually depressed for one to three days during which time there can be a substantial inotrope requirement. Regression of the diastolic volume is more marked than that of the endsystolic volume, leading to a decreased right ventricular ejectionfraction. All patients had a normal global left ventricular function with a median ejectionfraction of 0.66 (0.59-0.72) before and 0.64 (0.58-0.7) after surgery. There was no significant change in ejectionfraction or tricuspid regurgitation. Cardiac performance, as assessed by the stroke volume, cardiac output, and ejectionfraction were reduced in the pathologic outcome group. The heart also improves in contractility, resulting in a larger ejectionfraction, a larger stroke volume and increased maximal cardiac output. Neither were significant differences seen in left ventricular percent fractional shortening, left ventricular ejectionfraction, and mean rate corrected velocity of circumferential fiber shortening. Where left ventricular ejectionfraction is less than 40%, we recommend abstinence from strenuous or competitive sports. The right ventricular ejectionfraction improved from 0.39+0.04 to 0.6+0.03 (p<0.001). These examples are from corpora and from sources on the web.
