A 22-year-old pregnant woman was referred for a detailed fetal echocardiogram because an abnormality of the fetal heart was suspected on routine antenatal scanning. A 29-year-old pregnant woman was referred to our hospital for a detailed fetal echocardiogram because of a fetal arrhythmia. A murmur was noted incidentally at the age of seven weeks, and the echocardiogram showed tricuspid atresia due to absence of the right atrioventricular connection. The echocardiogram demonstrated an atrial septal defect within the oval fossa and normal contractility. Among patients who are followed with frequent echocardiograms, new lesions are virtually never identified beyond four weeks after the onset of the disease. Patient records, catheterization data, cineangiograms and the most recent echocardiograms at latest follow-up were reviewed. An echocardiogram showed a large solid mass with cystic components anterior and superior to the heart within the pericardium. In retrospect, however, the anatomy could be distinguished on preoperative echocardiograms. Fetal echocardiograms were repeated at 26 weeks and 35 weeks of gestation. The next echocardiogram was performed at the 33rd week of gestation. This was despite two separate echocardiograms in two different centers, both performed by experienced echocardiographers but using portable equipment. This course within the left atrial wall (truly intramural) could not be seen even on retrospective evaluation of the echocardiograms and angiograms. An echocardiogram at that time revealed absence of the right pulmonary artery. We thank our many colleagues for letting us use the echocardiograms of patients under their care. The postnatal echocardiogram confirmed the fetal diagnosis, showing a bicuspid aortic valve with an eccentric orifice. For each patient, the post coil(s) placement angiogram, early post procedural (within 8 hours) echocardiogram, and follow up echocardiograms were reviewed. A repeat echocardiogram showed a large atrial septal defect and decreased left ventricular function. Since then, her left ventricular function on serial echocardiograms continued to improve. A further echocardiogram on day 29 demonstrated reduction in the degree of tricuspid insufficiency and of the right atrial dimensions. At follow-up of 42 months, the patient has remained asymptomatic and the echocardiogram shows mild tricuspid regurgitation that has not changed since surgery. Serial fetal echocardiograms were performed, and the infant was subsequently delivered in a high risk referral center with neonatologists and pediatric cardiologists in attendance. The echocardiogram showed bowing of the ventricular septum from left to right with normal ventricular shortening fraction (29%). The echocardiogram provides the most valuable information concerning functional and morphologic aspects. Evaluation prior to surgical cannulation requires an expert pediatric cardiological opinion, which should always include evaluation of an echocardiogram.
