The balloon expandable intravascular stent is designed to support the vascular walls when the stenotic shape recurs immediately after a balloonangioplasty procedure. It is loaded onto a standard balloonangioplasty catheter and manipulated into the stenotic area. Of the patients, 13 had no significant stenosis, while 25 had significant stenoses and, therefore, underwent balloonangioplasty. Success rate of balloonangioplasty for pulmonary artery stenosis was 50% (19 of 38 dilations). In individual cases, nonetheless, localized stenoses appeared to be effectively dilated as evidenced by the angiographic findings after balloonangioplasty. Three of these 5 patients were redilated successfully by second balloonangioplasty. Aortic constriction was treated with balloonangioplasty (6), surgery then balloonangioplasty (4), or surgery alone (1). Complications with balloonangioplasty were need for transfusions in two and femoral arterial compromise in two. Now that closure methods using a catheter, like balloonangioplasty, are confronting common anomalies, they are occupying more of the energies of the pediatric cardiologist. The recent introduction of balloonangioplasty provides yet another palliative option for young symptomatic infants presenting prior to the optimal age for primary repair. Percutaneous balloonangioplasty for recoarctation of the aorta is immediately effective in reducing pressure gradients. Sequential balloonangioplasty then permitted gradual enlargement of the newly created channel, and subsequent placement of stents established a persistently wide venoatrial communication. Balloonangioplasty (balloon diameter 15 mm) of the stenosis in the left pulmonary artery was unsuccessful. All patients had imaging studies before and after balloonangioplasty. In most children this can be treated successfully with balloonangioplasty. Other methods of promise include the use of stents to maintain patency of vessels which do not respond to balloonangioplasty. This stent procedure may be applicable to small infants with branch pulmonary artery stenoses, obviating the need for early surgical or balloonangioplasty techniques. Also, both balloonangioplasty and valvuloplasty procedures can be performed effectively to the patients with aortic coarctation and stenosis.
