Postural hypotension and anticholinergic side-effects, including confusion, are common with low potency neuroleptics such as thioridazine and chlorpromazine. The first was from the discovery of chlorpromazine to the early 1960s. All patients were sedated with a mixture of meperidine, promethazine and chlorpromazine. The most commonly-used neuroleptics include haloperidol, thioridazine, and chlorpromazine. Patients prescribed depot medication received on average a lower daily dose in chlorpromazine equivalents than those prescribed oral medication. Antipsychotic dosage was converted to milligram equivalents of chlorpromazine and antipsychotic type (typical v. atypical) was noted. These differences cannot be explained by differences in medication as the median dose for the two groups in chlorpromazine equivalents was the same. Chlorpromazine is the most important single psychotropic drug in the group. Alternatively, the benzodiazepines and low-potency conventional neuroleptics such as chlorpromazine may be associated with sedation or falls. It is important to note the anticholinergic and hypotensive side effects of chlorpromazine, particularly in the elderly patients. Such elements of happy accident as attended the evolution of chlorpromazine have by no means always been the case. In addition, both haloperidol and chlorpromazine were demonstrated to significantly improve the symptoms of delirium in both the hypoactive and hyperactive subtypes. An alternative strategy in agitated patients is to switch from haloperidol to a more sedating antipsychotic such as chlorpromazine. |
