The patient could not tolerate loud sounds due to paralysis of the stapedius muscle that contracts and holds back the stapes if the ear is exposed to loud sound. Hyperacusis occurs if the nerve to the stapedius is involved. Acoustic reflexes are the bilateral contraction of the stapedius muscles in response to loud sounds. The stapedius muscle is the smallest muscle of the human body, measuring approximately 6 millimetres in length.
