In some cases of complete transposition, both arterial valves are supported by complete muscular infundibulums. It also appears that on the whole the patients with poorly formed infundibulum have small tricuspid valves. The muscular outlet septum, and the "septal" components of the subpulmonary infundibulum, were absent. With a hypoplastic, or absent infundibulum, the pulmonary trunk is tapered, and there is usually an element of muscular atresia in the infundibulum as well. These are supported by the muscular infundibulum of the right ventricle. It is interesting, however, that they measured the diameter of the infundibulum during systole and could, therefore, have used undersized balloons. Another reason for not using the video images was the detection of marked changes in the width of the pulmonary infundibulum throughout the cardiac cycle. In addition, there was endocardial sclerosis in the infundibulum, consistent with turbulent blood flow. The aorta takes its origin from this ventricle, arising via a muscular infundibulum. The posterior infundibulum was obstructed in six patients (22.2%), this being connected with the pulmonary trunk in four and with the aorta in two. The other three patients did not undergo surgery and, in these, the diagnosis was confirmed at autopsy and had side by side infundibulums. In 14%, right ventricular outflow tract creation would afford additionally stent implantation in the right ventricular infundibulum. Pulmonary atresia may be localised to the valve itself, or may additionally involve the infundibulum. A small opening was seen in the floor of the right ventricular infundibulum communicating with the right ventricular apical component.
