A heartmurmur was heard at ten days of age. A congenital cardiac anomaly was thus far unsuspected, even though a soft systolic heartmurmur had been known to exist since early childhood. She had cyanosis, clubbing and a grade 2/6 systolic heartmurmur. She developed fever within the month, along with frontal headache and a new heartmurmur. Firstly, during the study period, we performed echocardiography in every neonate who had a heartmurmur prior to discharge from the hospital. Examination at that time revealed a soft heartmurmur. All patients were asymptomatic and referred to the cardiology service for assessment of a heartmurmur. Fifteen patients had a heartmurmur, 11 of which were continuous. All patients initially presented with either congestive heart failure, cyanosis, or a heartmurmur. The heartmurmur disappeared completely after the operation. The commonest presentation was cyanosis (86%) but included cardiac failure(2), heartmurmur(l) and arrhythmia/heart block(l). In older children, symptoms are less severe and the syndrome is usually detected when investigation is undertaken for recurrent pulmonary infections, chest x-ray abnormalities or a heartmurmur. The most common presenting initial clinical signs were very non-specific - fever (52 cases), anemia (39 cases), new heartmurmur (39 cases) and enlarged spleen (17 cases). We report here an unusual case where persistence of a right cervical aortic arch, together with anomalous origin of the left subclavian artery, led to stridor and a heartmurmur.
