Rapidly progressive focal necrotising glomerulonephritis is common, and a leading cause of mortality. Pulmonary haemorrhage with respiratory failure may be present in either of the three forms of crescentic glomerulonephritis discussed above. The appearances were those of a mesangio-proliferative glomerulonephritis. Crescentic glomerulonephritis should be considered in any elderly patient presenting with acute renal failure associated with haematuria, red cell casts and proteinuria. Renal complications included acute renal failure, defined as an increase of serum creatinine by >0.5 mg/dl (44 mmol/l) during admission, or glomerulonephritis. Two patients were diagnosed with other forms of glomerulonephritis and long term follow-up was not provided for any of the patients. However, 99 % of the acute glomerulonephritis admissions were to fully enumerated hospitals, and therefore the influence of sampling would be small. Similarly, an immune complex mechanism is most probably responsible for the acute arthropathy and may also be the cause of more unusual manifestations such as glomerulonephritis and brachial plexus neuropathy. These autoimmune-mediated complications follow a small percentage of infections and include rheumatic fever and acute postinfectious glomerulonephritis.
