We report an unusual case of intrapericardial teratoma in a neonate presenting with collapse of the lung which was successfully treated by surgery. Histology showed the mass to be a benign immature teratoma. The pericardium has been opened and the large teratoma (marked by white dot) can be seen to displace grossly the heart and the great vessels. We present three cases of intrapericardial teratoma occurring in neonates. Typically, intrapericardial teratomas are located anteriorly and to the right of the heart and great vessels, often displacing the heart leftward and posterior. Because of this observation, the diagnosis "cystic teratoma" was considered most likely and ultrasonographically guided aspiration was performed.
