To elicit Babinski's reflex, the nurse strokes the sole of the patient's foot with a moderately sharp object, such as thumbnail. Babinski's sign is the earliest to occur in the presence of upper motor neuron pathology. Myelopathy causes severe sensory loss of posterior column sensation, spasticity, hyperreflexia, and positive Babinski reflexes. Upper motor neuron disease is characterized by spastic paralysis, hyperreflexia, and a positive Babinski reflex. Examination may reveal a positive Babinski sign and hyperreflexia, consistent with cerebral oedema.
