Ninety-five percent of the patients for whom revascularization was preferred received the treatment. The analysis of cardiac revascularization procedures was limited to the patients who underwent coronary angiography. In the case of coronary bypass surgery and arterial revascularization, nearly 58% and 56% (respectively) of respondents would, if necessary, lie. This amounts to an expense of approximately $23,000 for each revascularization procedure avoided. The odds of revascularization for persons without health insurance are 62.4% lower than persons with private insurance. The influences of gender and neighborhood-income on the odds of revascularization are, however, both significant and large. The analysis of cardiac revascularization procedure was limited to patients undergoing coronary angiography. At least one of the following outcomes had to be reported: death, myocardial infarction, angina pectoris, revascularization. Revascularization appears not to decrease the risk of myocardial infarction. The frequency of revascularization was two times greater among patients with recurrent ischemia than among other patients. This cost was divided by the number of avoided revascularization interventions for each scenario to calculate the cost per avoided revascularization intervention. The site of the coronary artery stenosis was the primary determinant of revascularization use. Compared with patients with chronic stable angina, patients with unstable angina were more likely and asymptomatic patients less likely to be recommended for revascularization. However, the study sample was younger, significantly more likely to have revascularization, and less likely to die. The second possibility was that clinicians may be reluctant to investigate female patients who are too sick to undergo revascularization. The relationship between categorical predictor variables and recommendation for coronary revascularization was analyzed with the chi-square statistic. Similarly, the density of physicians is not related significantly to revascularization. The determinants of revascularization use among patients undergoing coronary angiography appeared more closely consistent with previously published guidelines (2). However, this would be associated with savings in repeat revascularization of $9.7 million leading to a net incremental cost of $35.2 million. Therefore, to arrive at a balanced decision, reliable comparative data on the health benefits and adverse effects of different revascularization procedures should be available (2). When invasive treatment is indicated, the selection of the appropriate myocardial revascularization technique can be challenging and is at least partly subjective. In order to clarify this issue, it is necessary to study the progress of patients from their admission with acute chest pain through potential revascularization. The probabilistic sensitivity analyses, thus, show that the result is quite sensitive to difference in the probability of revascularization. Physicians from these specialties have been shown to differ in their assessment of the appropriateness of theoretical indications for coronary angiography and coronary revascularization (6).
