The patient had absence of the right atrioventricular connection, with concordant ventriculo-arterial connections, and naturally occurring pulmonary stenosis. A long non-branching coronary artery emerges from the left-hand-facing sinus (sinus 2) and runs behind the pulmonary trunk to enter the left atrioventricular groove. Aliasing of flow was seen from the left pulmonary veins entering the left atrium, suggesting obstruction. The malaligned atrial septum was widely excised to reveal a small left atrium receiving the pulmonary venous orifices and a tiny atrial appendage. At operation, severe obstruction at the site of entry of the pulmonary veins into the left atrium was found, more marked on the right side.
