The present study was undertaken to investigate the effects of inspiredoxygen and positive end-expiratory pressure on arterial blood gases and systemic delivery of oxygen. As expected, both increasing positive end-expiratory pressure and decreasing fraction of inspired oxygen increased pulmonary artery pressure and pulmonary arterial vascular resistance. Following treatment with intravenous antibiotics and supplemental oxygen, oxygen saturation measured transcutaneously remained between 88 and 90%, even with administration of 100% inspiredoxygen. Low levels of inspiredoxygen, however, decrease the delivery of oxygen to the tissues. A number of combinations of positive endexpiratory pressure and levels of inspiredoxygen produced similar changes in pulmonary vascular resistance. We showed that a low level of inspiredoxygen, and positive end-expiratory pressure, both increased pulmonary vascular resistance, and that their effects were additive. The present study investigated the effects of inspiredoxygen and positive end-expiratory pressure on pulmonary vascular resistance. At all levels of positive pressure ventilation, a fraction of 0.15 of inspiredoxygen increased pulmonary vascular resistance. In the present study, both an increase in positive end-expiratory pressure and a decrease in inspiredoxygen increased pulmonary vascular resistance. Increasing the concentration of inspiredoxygen produces a reduction of the right-to-left shunt and, in our case, normalised the aortic saturation during the uterine contractions. In intact neonatal pigs, we studied combinations of fraction of inspiredoxygen and positive end-expiratory pressure. The oxygen delivery was impaired only at a level of 0.15 for inspiredoxygen. This was not significantly different on 100% inspiredoxygen. Neither systemic arterial pressure, cardiac output, or heart rate were altered significantly at any combination of positive end-expiratory pressure and fraction of inspiredoxygen. Blood gas samples were obtained at each level of inspiredoxygen. Statistical significance for pulmonary vascular resistance and oxygen delivery was determined by a two-way repeated measures analysis of variance (one way for ventilation; the other way for inspiredoxygen fraction). The rate and inspiratory pressure during negative pressure ventilation were adjusted to maintain similar end-tidal carbon dioxide readings, and the inspiredoxygen concentration was not changed. At all levels of inspiredoxygen (1.00, 0.21 and 0.15), ventilation at a pressure of 15 cm of water increased pulmonary vascular resistance. The systemic delivery of oxygen, however, was found to be approximately 30% higher using the combination of high positive end-expiratory pressure and high level of inspiredoxygen.
