Local studies, of older people who have been followed over time as the provision of homehelp has changed, support these conclusions. Thus, they observed that older people who lived alone were five times more likely to receive homehelp support than those who are married. The comments made about the homehelp service were negative or, at best, neutral. The centres also offer homehelp services with older people contributing to the development of care at home. Only people with severe disabilities, low incomes and with no direct family caregivers, may receive homehelp. In recent years, fewer people have received homehelp, and this help is now centred primarily on personal care, and less on household tasks. Despite these problems, homehelp is a valuable but under-used alternative to institutional care in both countries. It may not be possible to offer homehelp for everyone with a need. This may include an orthopaedic surgeon, geriatrician, general practitioner, day hospital nurse, community nurse, occupational therapist, physiotherapist, social worker, dietitian, orthotist and homehelp. Limited availability and accessibility of home nursing and homehelp services make close family members the main care givers, above all partners, daughters and daughters-in-law. Most of the nonhospital resources were used for homehelp. Among the patients, 31% had homehelp related to the stroke, with a median of 76 (extra) hours during the year. In total, 82% of those who had a homehelp named that person as their confidant. But this will not be enough to cover the increasing demand, and homehelp was considered a cheaper alternative. To older people, interviewed in the course of this research, the homehelp service embodied this service. Given that staff and labour constitute the main inputs, we assumed a unit cost equal to 15 min of homehelp. The most important resource categories were the primary hospital stay, readmissions to hospital, nursing homes, outpatient rehabilitation, and homehelp. Homehelp had clearly been targeted at more needy individuals. For 20 years she had worked as an assistant administrator of the older people's homehelp service in a social welfare office. When such requests are accounted for, the requested task is treated as something that should be routinely performed, and hence as something the homehelp assistant has neglected to do.
